In addition to the online application, we require the following materials:
An optional Academic Statement of no more than 250 words explaining any academic issue in your record.
3 Letters of Recommendation that speak to your academic potential and career goals; we accept letters from Interfolio sent to email@example.com
Unofficial Transcripts that report all of your college level coursework from all institutions. Official transcripts are acceptable but not necessary.

Curricula/Vitae(CV)/Resume to show us your professional activities during and after college
Standardized Test Scores: We require one of the following: GRE general exam, MCAT, DAT, or PCAT.
GRE scores should be sent to Institution Code 1105 through ETS. We can then download your scores from ETS.
We require a Verified Score Report from AMCAS. Please DO NOT have AMCAS send your MCAT scores to CWRU--those will go to the Medical School and we will not receive them. Please log into AMCAS and print a Verified Score Report to a pdf file and email that to us at firstname.lastname@example.org. This report will show your MCAT score, your name and AMCAS ID number, and a Verification Code. We must have this report for your application to be complete.
DO NOT take the MCAT solely to apply to this program--take the GRE instead.
We recommend that incoming students have one year of English, one year of biology, one year of physics, and two years of chemistry (through organic chemistry) on their undergraduate record.
We review each applicant holistically and have no absolute criteria for selecting applicants based on standardized exams or undergraduate grade point averages. We are reviewing your application to see if you can be successful in our program.
